Overview
Fetal anomalies are structural or functional abnormalities of the developing fetus that arise during gestation, ranging from isolated organ malformations to complex multisystem syndromes. They are classified by mechanism and timing into malformations originating in faulty organogenesis, deformations from mechanical constraint, disruptions from interruption of normal development, and dysplasias of tissue organization, and they may be sporadic, chromosomal, single-gene, or environmentally influenced in origin. Prenatal detection relies on imaging, principally targeted two- and three-dimensional ultrasound supplemented by Doppler interrogation of blood flow and, where indicated, fetal magnetic resonance imaging, allowing characterization of conditions such as congenital diaphragmatic hernia, thoracic and abdominal cystic lesions, pleural effusions, and disorders of laterality including left isomerism. Accurate prenatal prognostication integrates anatomical findings with measured indices of severity to predict postnatal outcome and to stratify which fetuses may benefit from intervention. A subset of anomalies is amenable to in-utero management, including shunt placement for fluid-filled compartments and other fetal-surgical procedures intended to halt secondary damage before birth. Comprehensive evaluation combines diagnostic imaging, genetic and biochemical workup, multidisciplinary counselling, and planning of delivery and neonatal care, with the goal of optimizing survival and reducing long-term morbidity in affected pregnancies.
