Overview

Chronic gastritis is long-standing inflammation of the gastric mucosa, defined histologically by the infiltration of inflammatory cells into the stomach lining and, over time, by changes such as glandular atrophy and intestinal metaplasia. It commonly results from persistent Helicobacter pylori infection, autoimmune mechanisms, chronic use of nonsteroidal anti-inflammatory drugs, or bile and chemical injury, and is classified by cause, location within the stomach, and the pattern and severity of mucosal change seen on biopsy. Diagnosis and subtyping rest on endoscopic sampling and clinicopathologic analysis; distinctive forms, such as granulomatous gastritis characterized in biopsy series, require careful histologic evaluation to identify their underlying etiology. Because the upper gastrointestinal tract functions as an integrated system, motility and reflux disorders of the digestive tract, and their surgical correction, intersect with gastric inflammation and its clinical manifestations. Infectious and oncogenic factors are also relevant, including the association of Epstein-Barr virus with gastric carcinoma, reflecting the potential progression from chronic inflammation toward malignancy. Clinically, chronic gastritis may be asymptomatic or cause dyspepsia, and it carries risks including peptic ulceration, vitamin deficiency, and, in atrophic forms, gastric cancer. Research themes include the histopathologic grading of mucosal injury, the roles of infection and autoimmunity, and the prevention of complications through diagnosis and targeted treatment.
