Overview
Thrombosis is the pathological formation of a blood clot, or thrombus, within a blood vessel or the heart, obstructing circulation at the site of formation or, if dislodged, producing embolism downstream. It is classically understood through Virchow's triad, the three interacting predisposing factors of venous stasis, endothelial or vascular injury, and hypercoagulability of the blood. Arterial thrombosis, typically platelet-rich and often superimposed on ruptured atherosclerotic plaque, causes myocardial infarction and ischaemic stroke, whereas venous thrombosis, characteristically fibrin- and red-cell-rich, manifests as deep vein thrombosis and pulmonary embolism. Inherited and acquired thrombophilias, including Factor V Leiden and other coagulation abnormalities, raise thrombotic risk, while malignancy and its treatment can disturb haemostatic balance. Thrombosis may also arise at unusual sites and in association with prosthetic surfaces or surgical reconstruction. Studies in this area examine postpartum ovarian vein thrombosis, jugular and cerebral venous sinus thrombosis, the failure of vascular grafts, hypercoagulability complicating acute leukaemia, inherited bleeding and coagulation disorders, and the role of Factor V Leiden. The field integrates haemostasis, vascular biology, genetics, and clinical medicine to understand clot formation, its predisposing conditions, and the spectrum of arterial, venous, and site-specific thrombotic disease and their sequelae.
