Overview

Primary open-angle glaucoma (POAG) is a chronic, progressive optic neuropathy and the most common form of glaucoma, characterised by gradual loss of retinal ganglion cells and their axons, structural changes at the optic nerve head, and corresponding visual field defects, occurring while the anterior chamber drainage angle remains anatomically open. The term primary indicates that it arises without an identifiable secondary cause. Elevated intraocular pressure, resulting from impaired aqueous outflow through the trabecular meshwork, is the principal modifiable risk factor, although the disease can also occur at statistically normal pressures, implicating additional vascular, mechanical, and genetic factors in optic nerve susceptibility. Genetic contributions, including variants in genes such as optineurin, are recognised in disease pathogenesis. Because it is typically asymptomatic until substantial and irreversible damage has occurred, POAG is a leading cause of preventable blindness, and detection depends on examination of the optic nerve, measurement of intraocular pressure, and functional testing of the visual field using perimetry, including techniques such as frequency-doubling technology. Management aims to lower intraocular pressure and preserve remaining vision through topical medications, laser trabeculoplasty, and surgical drainage procedures. Public understanding and glaucoma literacy also influence timely diagnosis, adherence to treatment, and long-term preservation of sight.
