Overview

Headache is pain perceived in any region of the head, and it is one of the most common neurological complaints. It is broadly divided into primary headaches, in which the headache is the disorder itself, such as tension-type headache and migraine, and secondary headaches, in which pain arises from an underlying condition such as vascular disease, infection, raised intracranial pressure, or structural lesion. Tension-type headache, the most prevalent primary form, is associated with muscular and cerebrovascular factors, while warning features may signal serious secondary causes requiring urgent evaluation. Diagnosis depends on careful history, examination, and, when indicated, neuroimaging to identify or exclude underlying pathology. Research relevant to this area includes brain hemodynamics and cerebrovascular reactivity in tension-type headache, ocular and chiasmal lesions presenting with headache and visual symptoms, lacunar infarction, reversible posterior encephalopathy syndrome, and other neurological disorders in which headache is a prominent feature. Management is tailored to the headache type and may involve acute symptom relief, preventive therapy, treatment of the underlying cause, and non-pharmacological measures addressing triggers such as stress, dehydration, and muscle tension. Because headache ranges from benign and self-limiting to a marker of life-threatening disease, accurate classification and recognition of red-flag features are essential to appropriate care.
