Overview

Carotid endarterectomy is a surgical procedure that removes atheromatous plaque from the inner lining of the carotid artery to restore lumen patency and reduce the risk of ischemic stroke arising from carotid stenosis. The carotid arteries supply the anterior cerebral circulation, and progressive atherosclerotic narrowing at the carotid bifurcation can shed emboli or critically limit perfusion. In the operation, the artery is clamped and opened longitudinally, the plaque and diseased intima are dissected away, and the vessel is closed primarily or with a patch graft, sometimes using a temporary shunt to maintain cerebral blood flow. Patient selection rests on the degree of stenosis and whether the lesion is symptomatic, with surgery weighed against carotid stenting and optimized medical therapy. Within neurological practice, the procedure sits among broader concerns of cerebrovascular disease, including stroke subtypes, endothelial dysfunction, carotid intima-media thickening, arterial dissection, and the management and outcomes of stroke inpatients. Perioperative considerations include controlling embolic risk, monitoring neurological status, and preventing complications such as restenosis, cranial nerve injury, or hyperperfusion.
