Overview

Glaucoma is a group of progressive eye diseases that damage the optic nerve, the structure that transmits visual information from the retina to the brain, and is a leading cause of irreversible blindness worldwide. Damage is frequently, though not always, associated with raised intraocular pressure (IOP), the fluid pressure inside the eye, and characteristically produces gradual, often asymptomatic loss of peripheral vision before central sight is affected. The two principal forms are primary open-angle glaucoma, in which the eye's drainage angle remains anatomically open but aqueous outflow is impaired, and angle-closure glaucoma, in which the drainage angle becomes obstructed, sometimes acutely. Because early disease is typically painless and unnoticed, detection depends on screening and clinical assessment, including measurement of intraocular pressure, examination of the optic nerve head, and functional testing of the visual field. Research published in this journal addresses several of these aspects, including public awareness and glaucoma literacy, the relationship between intraocular pressure and the optic nerve, visual-field and perimetric assessment, and the broader effect of visual impairment on quality of life. Related work touches on optic nerve head drusen, retinal vascular disease, and ocular manifestations of systemic and inflammatory conditions. Management generally aims to lower intraocular pressure, slow optic nerve damage, and preserve remaining vision, since lost sight cannot be restored.
